To keep things running smoothly, arrive fully prepared with all required documents and materials. What to Bring 1. Completed Application Download the DS-11 Passport Application here . Complete the DS-11 Passport Application but DO NOT SIGN. 2. Proof of U.S. Citizenship Certified U.S. birth certificate with: Official seal Full name, date of birth, place of birth Parent(s)’ names Filing date within one year of birth Registrar signature OR Full-validity, undamaged U.S. passport 10 years (adults) 5 years (minors) Consular Report of Birth Abroad or other U.S. Department of State birth document Certificate of Naturalization or Citizenship. 3.
